Food Distribution Program Application - Download ➤
Declaratory statement of eligibility, All pre-registering households must complete an Application/Declaratory Statement of Eligibility. An application must be approved and on file in order for the household to receive commodities. A Food Pantry must adhere to the eligibility requirements within this document for distributing donated food to needy households and must keep on file a completed Application/Declaratory Statement of Eligibility for the applicant.

Service Insights refers to the collection of basic, administrative data from the people we serve during intake at a food program; also known as electronic client data collection. Using a common software system - MealConnect - across agencies helps standardize the data that is collected during client intake. Monthly Report Page 1 (LANIAP) - Download ➤
In order to receive LANIAP foods, member agencies must determine eligibility of households, seniors and children. Your assistance getting the information requested below to the Food Bank will help insure continued funding for this program and your organization’s continued access to these nutritious foods. Completed forms are due to the Food Bank of Central Louisiana no later than the 10th of each month.
Monthly Report Page 2 (Client Count Worksheet/Pantry Report) - Download ➤
This form can be used to track your client count (households, seniors, adults, and children) This form is for your convenience and does not need to be returned to the Food Bank.
Pest Control/Cleaning Log - Download ➤
Monthly pest control, professionally or privately sprayed, must be documented in a written log with the date and the full signature of the person who treats the premises. Wiping of the shelves at least once a month is recommended.
Temperature Log - Download ➤
Food should be stored at safe temperatures. A daily check on temperatures is encouraged, but at least three times a week is a must. Please date and use full signature. Remember 34 to 40 degrees for the cooler and 0 degrees for the freezer.
Distribution Rates Form - Download ➤
A copy of the distribution rates must be provided to the Food Bank and the original should be kept in your files and be available for review by Food Bank staff or LDAF representatives.
